Care Coordination At Beloit Memorial Hospital, we are committed to providing exceptional, patient-centered care that supports healing and improves outcomes. We are seeking a compassionate and skilled Care Coordinator / Inpatient Social Worker to join our interdisciplinary healthcare team. In this role, you will advocate for patients and families, help navigate complex healthcare needs, and facilitate safe, effective transitions of care. Conduct comprehensive psychosocial assessments to identify patient and family needs. Develop and implement individualized discharge plans to support safe transitions of care. Coordinate community resources, post-acute services, and supportive care programs. Provide counseling, emotional support, and crisis intervention to patients and families. Collaborate with physicians, nursing staff, case managers, and interdisciplinary teams to develop patient-centered care plans. Assist with complex cases involving abuse, neglect, domestic violence, homelessness, substance use disorders, mental health concerns, and financial hardship. Facilitate referrals to rehabilitation facilities, skilled nursing facilities, home health agencies, hospice services, behavioral health programs, and community organizations. Educate patients and families about available resources, treatment options, and discharge plans. Advocate for patient rights and assist with advanced directives, guardianship concerns, and ethical care issues as appropriate. Document assessments, interventions, discharge plans, and patient progress in the electronic medical record. Maintain knowledge of regulatory requirements, hospital policies, and community resources. Job Requirements Minimum of Bachelor's Degree in Social work (Master's Degree preferred). Certification to practice as a social worker in the State of Wisconsin (minimum of temporary certification). Knowledge of discharge planning, care coordination, and community resources. Strong communication, assessment, and crisis intervention skills. Ability to collaborate effectively within a multidisciplinary healthcare team. Conflict resolution and de-escalation Empathy and cultural sensitivity Strong organizational and documentation skills
