32 hours/weekFTE Status:
0.80FTE, Full-Time Work Schedule:
Mix of day and p.m. shifts with an every other weekend rotation; Day shift- 7:00am

Job Description:
Responsible for case review and telephone communications with patients for assessment, and assistance with appropriate level of care needs and/or safety planning if appropriate. Responsible for answering phone calls, telephone assessment and triage, and scheduling full patient assessments with appropriate level of care resource. Functions as a member of the multi-disciplinary treatment team. Responsible for communications across entire health system related to patient referrals and outcomes.Qualifications:
Bachelor's degree in Social Work, Psychology, Counseling, or related field required. Previous mental health triaging experience required.
