West Allis, WI | Full-Time | Flexible Hours | Hybrid Work Option Available Join our passionate team at PSG/CIP, a dynamic and community-focused social services organization providing innovative mental health programs across eastern Wisconsin! Position Overview We are seeking a compassionate and driven Care Coordinator to join the Adult Comprehensive Community Services (CCS) Care Coordination program with Professional Services Group in Milwaukee County. In the Care Coordinator position, you will support individuals with significant mental health and substance use challenges. You'll deliver wraparound, strength-based services that empower clients in their homes and communities, using a recovery-oriented and person-centered approach. Key Responsibilities Conduct comprehensive strengths and needs assessments during intake and throughout services Develop and implement individualized Recovery Plans of Care Coordinate and lead team meetings with service providers and support networks Provide crisis intervention and safety planning as needed Attend court hearings, psychiatry appointments, and provide advocacy support Safely transport clients to appointments when necessary Build strong formal and informal support teams
Position Details Typical Hours:
Monday-Friday, 9:00 AM - 5:00 PM (some flexibility)
Travel:
Local travel required; mileage reimbursement provided
Work Setting:
Hybrid/remote scheduling available
Pay:
Bachelor's Degree:
$23.00/hr •
Master's Degree:
$26.00/hr What You'll Need Bachelor's Degree in Social Work, Psychology, Criminal Justice, or related field (required) Valid driver's license, reliable daily transportation, and insurance 1+ year experience working with individuals with mental health or substance use needs (preferred) Why Work with PSG/CIP?
