2989949115-2 Apply for this job now Job Description Humana is seeking a Case Manager to support members across Green Lake, Marquette, Adams, and Waushara counties. In this role, you will coordinate integrated care for individuals with diverse medical, behavioral, and social needs. You will complete comprehensive assessments, develop and update person-centered care plans, and connect members with providers and community resources. Working in a member-focused, collaborative culture, you'll educate members on benefits and treatment options, monitor outcomes, and ensure regulatory compliance. Responsibilities Coordinate and manage care plans for members across Green Lake, Marquette, Adams, and Waushara counties Conduct assessments to identify medical, behavioral, and social needs Collaborate with physicians, social workers, and community resources to ensure integrated care Educate members and families on treatment plans, benefits, and available services Monitor progress, adjust care plans, and document all interactions in case management systems Advocate for members to reduce barriers to care and improve health outcomes Track quality metrics and ensure compliance with Humana and regulatory guidelines Participate in team meetings, trainings, and continuous improvement initiatives Required Skills Case management Clinical assessment Care planning Electronic health records (EHR) documentation Medicare/Medicaid knowledge Community resource coordination Motivational interviewing Data and outcomes tracking Regulatory compliance Member education and advocacy
