Job Description

We are seeking a dedicated and compassionate Qualified Developmental Disabilities Professional (QDDP) to join our dynamic team. In this vital role, you will provide specialized support and advocacy for individuals with developmental disabilities, ensuring they receive personalized care that promotes independence, community integration, and quality of life. Your expertise will help develop and implement individualized treatment plans, coordinate services, and foster positive outcomes for those we serve. REQUIRED KNOWLEDGE, SKILLS AND ABILITIES
: Must be highly organized and detail-oriented; excellent written and oral communication skills; ability to effectively advocate for individuals receiving services; knowledge of person-centered planning, HCBS requirements, Medicaid regulations, behavior support principles, risk assessment, quality assurance, and documentation requirements; ability to maintain confidentiality; other requirements as established by ADMH, Medicaid standards, and agency policy. Responsibilities Ensures the health, safety, dignity, privacy, rights, and overall well-being of individuals receiving services and supports freedom from abuse, neglect, exploitation, mistreatment, coercion, and retaliation. Coordinates PCP meetings, interdisciplinary team meetings, and other support team meetings as required. Assists in the development and revision and oversees the implementation and monitoring of Person Centered Plans (PCPs) to ensure services reflect individual strengths, preferences, goals, assessed needs, and desired outcomes. Ensures individuals, family members, guardians, advocates, and other team members are provided opportunities for meaningful participation in the person-centered planning process. Oversees development and implementation of training programs and support strategies designed to help individuals achieve personal goals and increase independence. Maintains and oversees all required individual records, assessments, documentation, and service-related information in accordance with regulatory requirements and agency policy. Completes quarterly reviews, progress reports, outcome evaluations, and other reports required by ADMH, Medicaid, and agency guidelines within established timelines. Verifies the accuracy and reliability of documented information through observation, record review, interviews, and monitoring activities. Completes annual functional assessments, reassessments, and other required evaluations prior to annual PCP meetings and as otherwise required. Reviews incident reports, investigates trends and concerns, makes recommendations, and facilitates team discussions regarding corrective actions, risk reduction, and prevention strategies. Oversees development, implementation, monitoring, and quarterly review of PMPs, BSPs, restrictive intervention reduction plans, and other specialized support plans as appropriate. Monitors service delivery to ensure supports are implemented as written and remain consistent with person-centered planning principles and regulatory requirements. Conducts routine observations and reviews to ensure quality services, regulatory compliance, and achievement of desired outcomes. Monitors individual rights protections and participates in the Human Rights Committee processes. Provides guidance, training, and support to staff regarding individual support needs, rights, preferences, communication methods, behavioral supports, and implementation of person-centered plans. Assists with audits, surveys, quality assurance reviews, corrective action plans, and continuous quality improvement activities. Collaborates with nursing staff, behavioral professionals, therapists, families, guardians, community partners, and other stakeholders to coordinate services and supports. Attends routine staff meetings, in-services, PCP meetings, Human Rights Committee meetings, and other meetings as required. Participates in professional development activities to maintain
ADMH QDDP
certification requirements and remain current on best practices, HCBS requirements, Medicaid standards, and regulatory changes. Performs other duties as assigned. Minimum Qualifications Bachelor's degree in a human service field (social work, psychology, sociology, nursing, or a related field) One or more years of experience working with persons with intellectual / developmental disabilities Must meet and maintain all ADMH requirements for QDDP certification and eligibility. Must be able to pass all required pre-employment screenings.
