Job Description
Signify Health is seeking an In-Home Physician to perform comprehensive, part-time in-home evaluations for Medicare Advantage and other vulnerable populations. You will conduct detailed medical histories, physical exams, and risk assessments, update medication lists, and identify care gaps to support value-based care. Using EHR and clinical decision tools, you will document findings and collaborate with interdisciplinary teams to improve outcomes.
