Responsibilities:
Collaboration and Communication:
- Foster effective communication among interdisciplinary teams to coordinate and enhance resident care.
Caseload Management:
- Manage resident caseloads with a keen focus on individual needs and behaviors, ensuring personalized care plans.
Meeting Facilitation:
- Lead engaging and productive meetings to discuss resident care plans, progress, and any necessary adjustments.
Compliance and Auditing:
- Conduct regular audits on Advanced Directives, Group notes, and documentation, ensuring compliance with regulations and maintaining high standards of care.
Training and Development:
- Assist in training and in-servicing staff members, fostering a culture of continuous learning and professional development.
Quality Assurance:
- Implement corrective actions and audits to maintain the highest standards of care, ensuring accuracy and completeness of resident records.
Admissions Oversight:
- Conduct audits on new admissions, facilitating seamless transitions and ensuring comprehensive follow-up.
Data Management:
- Monitor MDS completion and other essential documentation, providing support and guidance as needed to maintain accuracy and timeliness.
Performance Monitoring:
- Regularly review and analyze monthly statistics, providing necessary follow-up for residents falling below benchmarks.
Resident Advocacy:
- Act as a compassionate advocate for residents, ensuring their voices are heard and their needs are met with dignity and respect.
Regulatory Compliance:
- Demonstrate knowledge of HIPAA, Department of Aging regulations, and Sub Part S requirements, ensuring adherence to all applicable guidelines.
Incident Reporting:
- Understand protocols for reporting incidents of abuse or neglect, prioritizing resident safety and well-being at all times.
Additional Responsibilities:
- Perform any other duties as assigned by department managers, contributing to the overall success of our community.
