A Clinical Case Manager coordinates medical care and provides advocacy to help people get the best and most affordable care. Helps patients navigate insurance and hospital bureaucracies and access information. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment and travel to different doctors' offices, hospitals or a patient's home.

Job Description

Under limited direction coordinates the plan of care for the patient and family through the continuum of care, in accordance with the CVRMC Clinical Resource Management Plan.
QUALIFICATIONS
Must have either a current Arizona RN license or Clinical Social Work certificate in good standing, BSN or CSW preferred Must have at least 5 years of clinical experience. Recent case management experience in a healthcare setting preferred. Certification in Case Management by a recognized national Board preferred, and will be required within three years of hire. Must be able to interact effectively with co-workers, patients/family, physicians, and other hospital personnel. Must be able to perform effectively under physical and mental stress, maintaining concentration during that duration. Maintains current Case Management certification ( once acquired), maintains CPR.
