Case management positions are within our Behavioral Health Home where we provide Supportive/ Connective case management services to our members. Case Management at CHR may look a little different--- with a high focus on providing Personal Medicine Coaching, forming connections to facilitate healing and more time with members on a monthly basis. We need your experience, but we also need your willingness to work hard, do it different and really demonstrate that Case Management Models of Care within a Peer Run Organizations work and lead to great outcomes for those we serve. The Rehabilitation Specialist within our Behavioral Health Home is responsible to complete intake and annual vocational assessments with our members, connect them to Voc- rehab services, update employment related service plans, make referrals and follow up with members to connect them in meaningful community-based activities such as referrals to Peer Run Organizations. A rehabilitation specialist helps members navigate behavioral health services, discuss personal interests, and connect with community support and social skill-building.Service Coordination:
Connect individuals with appropriate behavioral health and support services;Skill Building:
Guide members in building critical social and daily living skills to help them live more independently;Care Team Collaboration:
Work alongside nurses, case managers, counselors, and peer support specialists as part of an integrated care team. The Substance Use Specialist on the ACT team requires at least 3 years experience providing supportive services to individuals with mental health and substance use disorders to include group facilitation. We prefer those with lived experience in this role. Additional training in substance use , motivational interviewing and stages of change is required . Experience working on an ACT team and vocational rehabilitation services is important . CHR serves individuals with serious mental illness and substance use disorders. As a Consumer Operated (Peer Run) organization offering services in a Behavioral health Outpatient Clinic, CHR aims to change how case management and clinic services are provided. We have a high focus on the use of personal medicine , being proactive , forming therapeutic relationships with all members and their extended families, spending less time responding to crisis and more time preventing it and creating a program that is welcoming , fosters a sense of belonging , care and compassion. CHR is a Peer Run Organization therefore we seek individuals who have lived experience as a priority. These positions are ideal for experience Peer Professionals looking to advance their career. Qualified candidates for our open positions would meet one of the following Bachelor's degree in a field related to Behavioral Health or Any other bachelor's degree and one (1) year of full-time behavioral health work experience or Any associate degree and two ( 2) years of full-time behavioral health work experience or Eighteen (18) credit hours of post-high school education in a field related to behavioral health and two (2) years of full-time behavioral health work experience or A high school diploma or GED and (3) years of full-time behavioral health work experience. Peer Positions require : Peer Certification and 2 years of full-time experience in behavioral health for CM Positions. Newly entering the field having completed Peer Training in an In - person recognized training agency will be considered for Entry level position Engagement Specialist positions. Peer Positions can also be case managers if you are experienced and have skills to advance into this position.P Experience:
Requires a minimum of 2 years of experience working with adults with a serious mental illness, demonstrating an understanding of the required knowledge, skills, and abilities. Willingness to be credentialed as Personal Medicine Coach and use different approach to Case Management. Previous experience working with SMI Care Management team is a plus Must be high energy, ready to work !Certification/Licensure:
Peer Certification required for Peer Support Position Requires possession of and ability to retain a current valid Arizona-issued driver's license in good standing and must carry the AZ legally always required minimum insurance coverage on a personal vehicle. must be at least 21 years of ageKnowledge, Skills, and Abilities:
Understanding of SMI system of resources and navigation . Understanding of trauma-informed care Knowledge and understanding of recovery-oriented service delivery. Must be able to work a flexible schedule in that member needs are a priority with understanding that those needs on occasion may extend beyond a normal shift as assigned . Requires the ability to have a reliable form of transportation to travel throughout the community with transportation of members as needed , company vehicles are available , must have clean driving record. Requires the ability to maintain highly confidential information, including a working knowledge of HIPAA. Must have the ability to work independently and seek supervision appropriately. Requires the ability to read, write, and speak effectively in English. Self-awareness, solid coping skills and hard work ethic are traits we seek for those that work at CHR. Applicants must meet the following qualifications/requirements: Must be 21 years or older. Pass a drug, TB, and background screen Must have the ability to exercise professional boundaries and ethics. Must have clean driving record, currently license , and own vehicle to provide community based services. Must have ability to type or document services in fast and efficient manner using our Electronic Health Record. Candidates with extended and solid recovery will be given preference. 1 plus years preferred. Candidates must be able to manage the stress and demands of working in the helping profession, with understanding our job is to provide support to those seeking our services. Must be reliable and able to attend work consistently. As a service provider regular and consistent attendance is an essential function of this position and directly impacts a person's confidence and sense of safety in the programs offered by CHR. Positions at CHR do not open up frequently. We want individuals who can make a commitment and who are truly motivated.Job Type:
