Job Description

Madison Avenue Support Services, Inc. is a small government contractor recruiting for
ACSW/AMFT
to provide case management and housing management services to Veterans. Please review and email info (at) Madison-support.com for more information/questions.
Note:
Clinical Hours for licensure will be provided.
Note:
If MSW/MFT, you MUST either have obtained or can obtain your
ACSW/MFT.
Note:
This is field based work. There is no remote option. Responsibilities Assess Veterans through comprehensive psychosocial evaluations to determine case-management needs, recovery goals and provide referrals to appropriate treatment or services. Work with the Veterans to obtain housing voucher from the PHA, provide housing search assistance, facilitate smooth move-in and ongoing case management to ensure housing retention. Employ Motivational Interviewing and Harm Reduction approaches Conduct regularly scheduled home visits Act as a liaison with VA and community resources, landlords, and PHA. Assist Veterans to identify housing goals, develop skills that increase self-sufficiency and eliminate barriers. Collect and submit
HUD-VASH
program evaluation data on Veteran participants. Identify opportunities to collaborative with local apartment complexes, rental agencies, and community services. Attend agency meetings and participate in community partner meetings as a Madison Avenue Support Services representative. Travel Required You will be expected to travel throughout San Francisco/Bay Area. Requirements Conditions of Employment Subject to a background/security investigation You must pass both the federal background check and VETPRO. Licensure, you must either have it or in the process of obtaining it from BBS.
Pay:
$40.85 per hour
