With a mission rooted in dignity, equity, and connection, PHC provides medical and behavioral health services at its clinic in Palo Alto, and community-based, case management and housing services throughout Santa Clara County through our New Directions Program. Summary Peninsula Healthcare Connection's New Directions program is currently recruiting an ECM Case Manager to provide community-based care coordination and case management services through our ECM program. ECM services address clinical and non-clinical needs of clients through intensive coordination of health and health-related services.

Case Manager Duties Will Include:

Provide in-person, community-based medical care coordination and case management services that are strengths-based, client-centered, and trauma-informed. In coordination with clinical ECM staff, complete comprehensive assessments to evaluate needs and create appropriate health action care plans. In partnership with clients, develop mutually agreed upon care plan goals and steps to be taken toward meeting those goals. Responsible for overall care coordination and communication between multidisciplinary providers to ensure provision of services and implementation of care plan. Make recommendations and referrals to governmental or community-based programs, benefits and services. Directly assist clients with linkage. Assist clients to access medical, mental health, and substance use treatment providers, including accompaniment to appointments as needed.

ECM Case Manager Requirements:

Bachelors degree in related field or equivalent relevant education and/or professional experience. Minimum two years experience working with populations experiencing barriers such as individuals experiencing homelessness, chronic medical conditions, substance use and/or mental health conditions. Demonstrated ability to develop collaborative partnerships with staff, clients, other professionals, and community members, including individuals from diverse cultural and socio-economic backgrounds. Valid CA driver's license, reliable transportation, and legally required insurance.

ECM Case Manager Qualifications and Skills:

Knowledge of client-centered practices to serve populations experiencing barriers including individuals experiencing homelessness, chronic medical conditions, and/or behavioral health concerns. Knowledge of community resources and benefit programs. Knowledge of Medicare, Medi-Cal, and managed care systems. Knowledge of the healthcare system in Santa Clara County, particularly safety net providers and services. Ability to communicate effectively, both orally and in writing. Ability to work effectively and efficiently with minimal supervision.

Pay:

$65,000.00 - $75,000.00 per year
