  • Are you passionate about helping individuals and families overcome barriers to healthcare and essential services? We are seeking a dedicated
  • Community Health Worker II (CHW II)
  • to serve as a trusted advocate, resource navigator, and liaison between patients, healthcare providers, and community services.
In this role, you will work directly with patients across clinics, community settings, outreach events, schools, mobile services, and other locations to connect individuals with healthcare, social services, and critical support resources.   • What You'll Do
  • As a Community Health Worker II, you will: + Serve as a trusted community advocate and liaison between patients, healthcare providers, and community organizations.
+ Conduct outreach, engagement, care coordination, and case management activities. + Complete Certified Enrollment Counselor (CEC) activities, including determining eligibility and assisting patients with health plan enrollment. + Analyze screening and assessment results and communicate findings to care teams. + Connect patients with healthcare services and community resources, including: + Food assistance + Housing resources + Financial support programs + Transportation assistance + Other social and healthcare services + Participate in multidisciplinary team meetings, case conferences, and training sessions. + Collect and maintain accurate documentation and data required by programs, funders, and regulatory agencies. + Support grant-related initiatives through eligibility assessments, socioeconomic screenings, and identification of healthcare disparities. + Assist individuals and families experiencing crises through advocacy, problem-solving, and resource coordination. + Utilize de-escalation techniques when appropriate. + Administer program evaluation tools such as surveys, pre-tests, and post-tests, and assist with outcome reporting. + Provide education and resources related to: + Nutrition + Physical activity + Personal hygiene + Substance use prevention + General health and wellness + Maintain confidentiality and accurate records of all patient interactions. + Work flexible schedules, including occasional evenings and weekends.

  • Required Qualifications
  • + High school diploma or equivalent + Community Health Worker (CHW) Certification + Certified Enrollment Counselor (CEC) Certification + 1-2 years of experience in community health, healthcare services, social services, or a similar field preferred + Bilingual skills are highly preferred + Strong communication, advocacy, and interpersonal skills + Ability to work effectively with diverse populations and community partners + Strong organizational and documentation skills •Work Environment•This position may involve: + Traveling between multiple sites and community locations + Visiting patients in their homes or accompanying them to appointments + Working outdoors in varying weather conditions + Providing services in clinics, schools, community events, mobile units, shelters, outreach locations, and other community-based settings + Flexible scheduling, including occasional evenings and weekends •Why Join Us?

  • Minimum Requirements:
