Job Description

Position Summary The Enhanced Care Management (ECM) Case Manager provides comprehensive, community-based care coordination to high-need Medi-Cal clients under the CalAIM initiative. This role focuses on delivering person-centered, trauma-informed services to individuals with complex medical, behavioral health, and social needs. The ECM Case Manager serves as the primary point of contact, coordinating across healthcare providers, social services, and community resources to improve health outcomes and reduce care fragmentation.
Supervision/reporting relationships:
The ECM Case Manger reports to the Program Director (and Associate Director if applicable). No supervision is given to other staff.
Minimum position requirements:
Coursework in psychology, social work, or related field preferred. One year experience in case management, care coordination, or social services working with high-risk populations. Knowledge and understanding of Medi-Cal and CalAIM Enhanced Care Management model and community based resources and service systems. Demonstrated ability to work independently as well as a team client. Ability to build rapport with diverse populations. Basic care coordination, organizational skills, caseload management, and problem solving abilities. Computer skills, including Microsoft Office 365 and electronic health record Must obtain a negative TB test or chest x-ray and clearance on an employment physical. Must be able to lift twenty pounds, view and utilize a computer, documents in writing and/or typed form. Hiring is contingent upon a full background screening in accordance with California licensing regulations; DOJ/FBI clearance, pre-employment physical and TB clearance. DMV printout and Basic First Aid certification is required prior to employment. Ability to travel locally for home and community visits. Key Responsibilities Care Coordination & Planning
  • Conduct comprehensive assessments of client's medical, behavioral health, and social needs
  • Develop and implement individualized care plans
  • Monitor progress and adjust care plans Client Engagement
  • Establish trusting relationships through culturally responsive approaches
  • Perform outreach in community settings
  • Support client self-management Interdisciplinary Collaboration
  • Coordinate with providers and community organizations
  • Participate in case reviews Resource Navigation
  • Connect clients to housing, food, transportation, and services
  • Advocate for services and benefits Documentation & Compliance
  • Maintain accurate, timely, and comprehensive documentation in accordance with CalAIM and Medi-Cal requirements
  • Document member encounters, care plans, and service activities to support ECM program compliance
  • Ensure all documentation meets standards required for CalAIM ECM billing, including service justification, time tracking, and encounter reporting
  • Track client progress, outcomes, and service delivery
  • Ensure compliance with confidentiality laws (HIPAA) and program guidelines Crisis Support
  • Identify and respond to urgent client needs and crises
  • Coordinate appropriate interventions and referrals
  • In conjunction with the Clinical Director, provide clinical supports as necessary to address complex or high-risk situations

Pay:
$30.00 per hour Expected hours: 40.0 per week
