Targeted Case Manager Face To Face Mental Health Services - 2.9 Miami, FL Job Details Full-time | Contract $30 an hour 2 hours ago Benefits Flexible schedule Qualifications Spanish Electronic health records (EHR) management CPR English Clinical case management Driver's License Bachelor's degree Working with individuals with mental health conditions Social Work Managing patients as a clinical case manager Full Job Description We are looking for a Certified Behavioral Health Case Manager (CBHCM) to provide case management services for children, adolescents, and adults. This role involves coordinating care, advocating for clients, and linking them to essential community resources.
Responsibilities:
Conduct initial assessments and develop service plans. Schedule and perform regular home and field visits. Maintain detailed documentation and progress notes. Advocate for clients and connect them with community resources. Communicate with psychiatrists, primary care physicians, and other professionals. Attend supervision, training, and staff meetings.
Qualifications:
Bachelor's degree in social work, psychology, counseling, or a related field. CBHCM certification from the Florida Certification Board (Required). Minimum 2 years of experience in mental health case management. Valid Florida driver's license, insurance, and reliable transportation. Bilingual (English/Spanish) preferred. Have their own clients (We take many insurances)

Up to $62,400 (based on a full-time schedule)
