Medical Case Manager Empower "U", Inc. - 4.3 Miami, FL Job Details Full-time $20.00 - $24.74 an hour 5 hours ago Benefits Paid holidays Paid time off Vision insurance Retirement plan Qualifications Bachelor's degree Full Job Description Position Summary Empower U Community Health Center is seeking a compassionate, organized, and patient-focused Medical Case Manager to provide comprehensive care coordination and support services to individuals living with HIV/AIDS and other vulnerable populations. The Medical Case Manager is responsible for assessing client needs, developing individualized care plans, coordinating medical and support services, advocating for patients, and promoting access to healthcare and community resources. The ideal candidate will have strong interpersonal skills, knowledge of community resources, experience working with diverse populations, and a commitment to improving health outcomes for underserved communities. Essential Duties and Responsibilities Case Management & Care Coordination Conduct comprehensive client assessments to identify medical, psychosocial, financial, housing, transportation, and support service needs. Develop and maintain individualized care plans based on client needs and program requirements. Coordinate access to medical care, specialty services, behavioral health, dental care, pharmacy services, and community resources. Assist clients with enrollment and recertification for Ryan White, ADAP, Medicaid, Medicare, and other benefit programs. Monitor client progress and update care plans as necessary. Provide crisis intervention, advocacy, and problem-solving assistance. Conduct follow-up activities to ensure continuity of care and service delivery. Assist patients in navigating healthcare systems and accessing available resources. Documentation & Compliance Maintain accurate, complete, and timely documentation in EPIC, PROVIDE, and other required systems. Ensure compliance with Ryan White Program requirements, HRSA standards, organizational policies, and regulatory requirements. Complete assessments, service plans, referrals, progress notes, and case closures within established timelines. Participate in audits, quality reviews, monitoring activities, and performance improvement initiatives. Patient Advocacy & Education Educate clients on available resources, treatment adherence, preventive health services, and self-management strategies. Promote patient engagement in medical care and support adherence to treatment plans. Serve as a liaison between clients, healthcare providers, and community agencies. Assist with reducing barriers to care, including transportation, housing, insurance, and social service needs. Team Collaboration Collaborate with physicians, nurses, behavioral health providers, peer educators, outreach staff, and community partners. Participate in interdisciplinary case conferences, team meetings, and program activities. Support organizational initiatives focused on quality improvement and patient-centered care. Perform other duties as assigned. Education Bachelor's Degree in Social Work, Psychology, Human Services, Public Health, Nursing, or a related field required. Master's Degree preferred. Experience Minimum one (1) year of case management, healthcare, social services, public health, or related experience required. Experience working with individuals living with
HIV/AIDS
preferred. Experience with Ryan White-funded programs preferred. Experience in a Community Health Center, Federally Qualified Health Center (FQHC), hospital, or healthcare setting preferred. Experience utilizing Electronic Health Records (EHRs) and case management systems preferred. Knowledge, Skills, and Abilities Knowledge of case management principles, practices, and community resources. Knowledge of Ryan White Program services and
HIV/AIDS
support programs preferred. Strong organizational and time-management skills. Ability to manage multiple client cases and competing priorities. Excellent communication, interpersonal, and patient advocacy skills. Ability to establish and maintain effective working relationships with clients, providers, and community partners. Ability to maintain confidentiality and comply with HIPAA requirements. Strong documentation and computer skills. Bilingual English/Spanish or English/Creole preferred.
