HIV/AIDS.
This position provides a range of client-centered services to link clients with health care, psychosocial, and other services identified during routine client assessments. The Medical Case Manager coordinates and follows-up with the client regarding medical treatment to ensure timely delivery of services, coordinated access and continuity of care. Key Responsibilities Conduct comprehensive client assessments and develop, implement, and regularly reassess individualized care plans to address medical and supportive service needs. Coordinate and monitor medical treatment, supportive services, and community resources to promote continuity of care and positive health outcomes. Provide treatment adherence and benefits counseling, helping clients access healthcare coverage, financial assistance, and other public and private support programs. Advocate for clients while fostering independence through education, navigation of healthcare and social service systems, and collaboration with healthcare providers and community partners. Maintain accurate and timely documentation, monitor service utilization, and conduct client visits to ensure ongoing engagement and quality care. Qualifications Bachelor's degree in human services or related field. Minimum one year of experience in case management, social services, or a related field. Experience working in the
HIV/AIDS
field preferred. Demonstrated ability to collaborate with colleagues and deliver respectful, high-quality care to patients in a community health setting across wide range of backgrounds and perspectives. Proficiency in Microsoft Office with strong attention to detail. Excellent communication, organizational, and problem-solving skills. Ability to maintain confidentiality and handle sensitive information with discretion.
