Job Description
Duties and Competencies:
Identifies and assesses case management needs and provides discharge planning to assure that the patient progresses through the continuum of care and is provided with an aftercare plan based on recommendations provided by clinical, medical and psychiatric providers.Facilitate group therapy in accordance with license requirements, AHCA, The Joint Commission standards and Futures of Palm Beach policiesDevelop and document a comprehensive clinical Discharge PlanCommunicates with referents.
Coordinates the facility activities concerned with case management and discharge planningProvide didactic/educational lecturesEffectively communicates relevant clinical information to the clinical team, patient and familyActively participates in treatment team meetings, providing relevant patient case assessmentDocumentation the progress and/or regression of the patient in an accurate and timely mannerRespond to and assist with crisis intervention and conflict resolutionDemonstrates ability to respond to feedback and supervision from supervisor and peersAdheres to Futures of Palm Beach, LLC policies and proceduresAdheres to departmental goals, objectives, standards of performance, and policies and proceduresMaintain appropriate boundaries, attendance, professionalism, and ethical behavior with patients, families, and other professionalsUphold strict patient confidentiality in accordance with Title 42 CFR, Part 2 and
HIPAA and Futures of Palm Beach Policies Qualifications:
Bachelors degree from an accredited school in behavioral sciences, counseling, or related human services.1 to 2 years-experience in an outpatient, day treatment, residential and/or hospital setting addressing mental healthPreferred Certified Behavioral Health Case Manager Benefits:
