We are seeking a compassionate, organized, dependable, and detail-oriented Clinical Assistant / Case Manager to join our clinical team. This individual will play an essential role in supporting daily clinical operations, coordinating client care, removing barriers to treatment, assisting with client engagement, and ensuring clients receive comprehensive support throughout their treatment and recovery journey. Position Summary The Clinical Assistant / Case Manager supports both the clinical and operational needs of Zenith Mental Health while serving as an advocate and resource for clients. This position works closely with therapists, medical providers, clinical leadership, administrative staff, clients, families, and community resources to promote treatment engagement and continuity of care. The Clinical Assistant / Case Manager will assist with admissions, client orientation, daily programming, attendance monitoring, case management, discharge planning, referrals, transportation coordination, documentation, scheduling, and communication with the treatment team. This position plays a critical role in creating a safe, organized, supportive, and client-centered treatment environment while maintaining professional boundaries, confidentiality, and compliance with organizational and regulatory standards. Primary ResponsibilitiesClinical Assistance & Client Support Provide daily support to the clinical team and assist with the coordination of client care. Support therapists and clinical staff with daily program operations and client needs. Assist with client check-ins, engagement, and communication throughout treatment. Help maintain a safe, structured, and supportive therapeutic environment. Assist clients with navigating the treatment program and understanding program expectations. Provide appropriate support to clients while maintaining professional boundaries. Facilitate or co-facilitate peer-led, educational, recovery, or supportive groups as assigned. Communicate relevant client concerns or changes in presentation to the appropriate clinical staff. Assist the clinical team with identifying barriers to treatment participation. Support clients in developing practical skills that promote treatment engagement and stability. Case Management & Care Coordination Coordinate client services from admission through discharge. Complete case management assessments and identify client needs. Develop and monitor individualized case management plans. Assist clients with accessing community resources, benefits, housing, employment, education, transportation, and recovery supports. Coordinate referrals to outside providers, community agencies, and supportive services. Assist clients with identifying and overcoming barriers that may interfere with treatment or recovery. Coordinate communication between clients, families, providers, and members of the treatment team. Assist with referrals when a client requires a higher or lower level of care. Track referrals and follow up to ensure continuity of services. Admissions & Client Orientation Assist with the admissions and intake process. Help orient new clients to the facility, programming, policies, and expectations. Assist with gathering and organizing required admission documentation. Ensure appropriate releases of information and required documentation are completed. Communicate client needs and relevant admission information to the clinical team. Help ensure a smooth and welcoming transition into treatment. Discharge & Aftercare Planning Assist with discharge planning beginning early in treatment. Coordinate aftercare appointments and referrals. Assist clients in identifying appropriate ongoing mental health, substance use, medical, and community-based services. Communicate with outside providers and referral partners to support continuity of care. Assist with transitions to higher or lower levels of care when clinically appropriate. Follow up on referrals and appointments to support successful transitions after discharge. Family & Client Support Maintain appropriate communication with family members and approved support systems. Provide families with information regarding treatment expectations and available resources as directed by the clinical team. Assist clients and families with navigating available community resources. Advocate for client needs while maintaining appropriate professional boundaries. Encourage treatment engagement, participation, and retention. Collaborate with therapists and other team members to address client concerns and barriers. Attendance & Client Engagement Monitor daily client attendance and participation in scheduled programming. Conduct outreach to clients who miss scheduled treatment when appropriate. Communicate attendance concerns with therapists, clinical leadership, and appropriate team members. Assist clients in resolving barriers that may impact attendance. Document client communication, outreach, and relevant interactions in the electronic health record. Encourage consistent participation in treatment programming. Transportation & Client Coordination Drive clients to and from treatment or approved appointments using the facility vehicle when needed. Maintain appropriate professional boundaries and safety standards while transporting clients. Assist with transportation coordination and scheduling. Communicate transportation concerns or barriers to the appropriate team members. Ensure transportation activities are documented according to organizational procedures. Documentation & Compliance Complete clinical assistant and case management documentation accurately and within required timelines. Maintain timely and thorough notes in the electronic health record. Assist with admission documentation and ongoing chart reviews or audits. Ensure releases of information and required client documentation remain current. Document referrals, outreach, appointments, and care coordination activities. Maintain strict confidentiality and comply with HIPAA and organizational policies. Follow all applicable regulatory, safety, and documentation requirements. Interdisciplinary Collaboration Participate in treatment team meetings and case consultations. Collaborate with therapists, medical providers, utilization review, administrative staff, and clinical leadership. Communicate important client updates and concerns to the appropriate members of the treatment team. Coordinate services with community providers, sober living programs, outside agencies, and referral partners. Support a collaborative and client-centered approach to treatment. Administrative & Program Support Assist with maintaining client schedules and appointment coordination. Help organize daily client programming and schedules. Assist with transportation and appointment coordination. Help monitor insurance authorizations and treatment timelines as directed. Track referrals and follow-up appointments. Assist with maintaining organized client records. Support daily operational needs to ensure efficient program flow. Assist clinical staff with administrative tasks related to client care. Perform other duties as assigned to support Zenith Mental Health and its clients. QualificationsPreferred Education Bachelor's degree in Social Work, Psychology, Human Services, Behavioral Science, Healthcare Administration, or a related field preferred. Equivalent behavioral health experience may be considered. Extensive experience in mental health, behavioral health, substance use treatment, or human services may be considered in lieu of formal education. Preferred Experience Experience in behavioral health, mental health, substance use treatment, healthcare, case management, or a related field. Experience working directly with clients experiencing mental health and/or substance use challenges. Knowledge of community resources and care coordination. Strong organizational and documentation skills. Excellent communication and relationship-building abilities. Ability to manage multiple priorities in a fast-paced clinical environment. Experience using electronic health records preferred. Valid driver's license and ability to meet organizational requirements for transporting clients, when applicable.
