Job Description

Atlas Behavioral Health is a growing behavioral healthcare organization dedicated to helping individuals recover from substance use and co-occurring mental health disorders. Our team provides compassionate, evidence-based treatment in a collaborative and supportive environment where clients are empowered to build lasting recovery. We are seeking a compassionate, organized, and detail-oriented Case Manager to join our clinical team. This individual will serve as an advocate for clients by coordinating care, removing barriers to treatment, supporting successful transitions, and ensuring each client receives comprehensive services throughout their recovery journey. Position Summary The Case Manager is responsible for coordinating client care throughout treatment by collaborating with clients, families, therapists, medical providers, and community resources. The Case Manager assists with admissions, discharge planning, attendance monitoring, documentation, referrals, transportation coordination, and communication between all members of the treatment team. This position plays a critical role in promoting treatment engagement, continuity of care, and positive client outcomes while maintaining compliance with organizational and regulatory standards. Primary ResponsibilitiesClient Care Coordination Coordinate client services from admission through discharge. Complete comprehensive case management assessments and identify client needs. Develop and monitor individualized case management plans. Assist clients with accessing community resources, benefits, housing, employment, education, and recovery supports. Coordinate referrals to outside providers and community agencies. Drive clients to and from treatment in the facility van. Facilitate peer-led groups. Assist staff with client coordination and referrals if a client needs a higher level of care. Admissions & Discharge Planning Assist with admissions and orientation to programming. Coordinate discharge planning beginning early in treatment. Arrange aftercare appointments and referrals. Ensure continuity of care through communication with outside providers. Support transitions to higher or lower levels of care when clinically appropriate. Family & Client Support Maintain communication with family members and approved support systems. Educate families regarding treatment expectations and available resources. Collaborate with the clinical team to address barriers affecting treatment participation. Encourage client engagement and treatment retention. Attendance & Client Engagement Monitor daily attendance and participation. Conduct outreach to clients who miss scheduled treatment. Communicate attendance concerns with therapists and leadership. Document all client communication in the electronic health record. Assist clients in resolving barriers impacting attendance. Documentation & Compliance Complete documentation accurately and within required timelines. Maintain case management notes in the electronic health record. Ensure releases of information and required client documentation remain current. Assist with admission documentation and ongoing chart audits. Maintain confidentiality and compliance with HIPAA and organizational standards. Interdisciplinary Collaboration Participate in treatment team meetings. Collaborate with therapists, medical providers, utilization review, and administrative staff. Coordinate services with sober living providers, outside agencies, and referral partners. Communicate important client updates to the treatment team. Administrative Responsibilities Maintain client schedules and appointment coordination. Assist with transportation coordination when needed. Help monitor client insurance authorizations and treatment timelines. Track referrals and follow-up appointments. Support operational needs to ensure efficient program flow. QualificationsPreferred Education Bachelor's degree in Social Work, Psychology, Human Services, Behavioral Science, Healthcare Administration, or a related field. Equivalent behavioral health experience may be considered or extensive experience working in the addiction field. Preferred Experience Experience in behavioral health, substance use treatment, mental health, or healthcare case management. Knowledge of community resources and care coordination. Excellent organizational and documentation skills. Strong communication and relationship-building abilities. Ability to manage multiple priorities in a fast-paced clinical environment. Experience using electronic health records is preferred. Valid driver's license with reliable transportation.
