Job Description

American Health Management, an EOE, has the following job opportunity available in Taylor and surrounding counties. Case Manager Join Horizon in helping clients remain in their homes and receive services in their communities as they age in place.
  • This position involves traditional case management duties for clients in the Home and Community Based Waiver.
  • Successful candidate will demonstrate strong communication and problem solving skills with a high degree of independent accountability and flexibility.
  • HCBW case management experience preferred.
  • Travel within Taylor, Green and surrounding areas required.
  • Required degree: Social Worker, RN, or Bachelors degree in health care related field.
