Hospital located near Winchester, Massachusetts is seeking full-time, permanent Case Managers to join their team!
Pay:
$45.00-$53.00/hourly
The Case Manager Job Details:
Full-time, direct hire position Mon-Fri, day shift, rotating weekends Great leadership and administration team! The Case Manager will collaborate with an interdisciplinary team to develop and guide treatment plans that align with patients' needs, goals, and preferences. Coordinate with team members to establish discharge plans and appropriate contingency strategies. Participate in the planning and implementation of the patient discharge process to ensure a smooth transition of care. Facilitate weekly team conferences, coordinate treatment plan updates and modifications, and complete case management addendums along with all required documentation in a timely manner.
The Case Manager Minimum Qualifications:
Must have active license as a RN, Social Worker, Occupational Therapist, Physical Therapist, Speech Therapist, or in Rehabilitation Counseling CCM or ACM certification required OR must be obtained within two years
