You'll collaborate closely with physicians, nurses, therapists, social workers, patients, and families to coordinate the entire rehabilitation journey. Responsibilities Collaborate with the interdisciplinary team to establish tentative discharge plans and appropriate contingency plans Coordinate and support the patient discharge experience from admission through transition Monitor patient experience, quality, timeliness, service appropriateness, and payer expectations Facilitate weekly interdisciplinary team conferences Coordinate modifications to patient treatment plans as needed Complete case management addendums and required documentation Ensure compliance with applicable regulations, standards, policies, and procedures Review and analyze case management reports and key care indicators Identify opportunities for improved outcomes and appropriate resource utilization Advocate for patients and help connect them with appropriate resources following discharge Qualifications Candidates must be qualified to independently complete assessments within the scope of their professional discipline, including: Registered Nurse (RN) Social Worker Occupational Therapist Physical Therapist Speech Therapist Rehabilitation Counselor Candidates must also meet eligibility requirements for CCM® or ACM™ certification upon entering the position or within two years of entry.
