SSM Health Physical Therapy - Day InstituteArnold, MOCase Manager - PRNExperience in physical medicine and rehabilitation strongly preferred.

SSM Health Day Institute is seeking a dedicated and compassionate Rehabilitation Case Manager to join our health care team, The ideal candidate will be responsible for coordinating and managing comprehensive rehabilitation plans for patients recovering from illness, injury or surgery. The role involves working closely with medical professionals, patients, and their families to ensure that each individual receives the appropriate care and support throughout their recovery journey. As a rehabilitation case manager, you will serve as a liaison between patients, healthcare providers, insurance companies, and community resources. Your goal is to facilities a smooth and effective rehabilitation process that promotes patient independence and quality of life. Key responsibilities include coordinating all therapy services, insurance authorizations and verifications, tracking visits and plan of care progression. The successful candidate will have strong organization and communication skills, a deep understanding of rehabilitation process, and the ability to work collaboratively in a multidisciplinary environment. Experience in case management, nursing, or therapy is highly desirable.
