Your Role at Sana Lake Sana Lake is expanding rapidly and is seeking an Outpatient Case Manager who is passionate about helping individuals and families recover! The Outpatient Case Manager works closely with the Clinical Operations Director to support daily operations and ensure high-quality, coordinated patient care. This role actively participates in daily huddles and census management meetings, collaborates with medical services staff to assist with day-to-day tasks, and facilitates the timely request, collection, and transmission of medical records. The Outpatient Case Manager coordinates care with outside providers and referral sources to promote continuity of care and positive patient outcomes. Additionally, this position contributes to team communication and program effectiveness through active participation in weekly staff meetings and ongoing collaboration with interdisciplinary team members. We would love to learn more about you if you: Bachelor's Degree in a related field, or a combination of an associate's degree and relevant experience to meet the required standard Minimum of 1 year of experience in substance use treatment preferred.
