ASC is seeking a compassionate, dependable, and highly organized Case Manager to provide comprehensive, client-centered case management to individuals and families participating in ASC programs. Position Summary The Case Manager works directly with clients to identify needs, establish goals, connect them with appropriate services and resources, and support progress toward improved health, stability, self-sufficiency, and overall well-being. This position requires more than providing referrals. Case Managers are responsible for actively managing an assigned caseload, maintaining regular client contact, monitoring progress, coordinating services, maintaining required documentation, following up on identified needs and referrals, and addressing barriers that may prevent clients from achieving their goals. The successful candidate will demonstrate initiative, sound judgment, strong organization, critical thinking, problem-solving, and consistent follow-through. Case Managers are expected to independently manage the routine day-to-day responsibilities of their caseload while recognizing when supervisory guidance or additional intervention is necessary. Key Responsibilities The Case Manager will: Conduct comprehensive client intakes, screenings, assessments, and reassessments to identify health, psychosocial, financial, housing, and other supportive service needs. Work collaboratively with clients to develop, implement, and regularly update individualized service plans with meaningful short- and long-term goals. Manage an assigned caseload and maintain current knowledge of each client's needs, goals, progress, barriers, outstanding requirements, and next steps. Maintain regular client contact based on program requirements, client needs, and level of support required. Monitor client progress and adjust service plans as circumstances, needs, and goals change. Coordinate referrals and assist clients in navigating healthcare, behavioral health, housing, public benefits, employment, transportation, and other community and supportive services. Follow up on referrals to determine whether clients successfully accessed services and provide additional assistance when barriers remain. Provide education, guidance, and support that promotes client stability, self-sufficiency, self-advocacy, and improved quality of life. Identify barriers affecting client progress and use available resources, critical thinking, and problem-solving to determine appropriate next steps. Respond appropriately to concerns or crises that may affect a client's health, safety, stability, access to care, housing, or continued participation in services. Advocate for clients when appropriate while helping clients develop the skills necessary to advocate for themselves. Conduct office, home, community-based, virtual, or other client visits as required by the assigned program. Promptly communicate significant client concerns, safety issues, compliance concerns, or changes in circumstances requiring supervisory attention. Participate in team meetings, case conferences, interdisciplinary coordination, training, and professional development. Collaborate with ASC staff, healthcare providers, housing providers, community organizations, and other service partners. Documentation & Caseload Management Accurate and timely documentation is an essential responsibility of this position.
The Case Manager will:
Maintain accurate, complete, and current client records in required electronic systems. Document client interactions, assessments, service plans, referrals, interventions, case-management activities, and outcomes. Maintain required eligibility documentation, reassessments, recertifications, and service-plan updates within established deadlines. Maintain an organized system for tracking client needs, appointments, referrals, documentation requirements, eligibility periods, deadlines, and required follow-up. Review assigned client records for completeness and promptly address missing, expired, inaccurate, or incomplete documentation. Ensure required documentation is completed before initiating services or assistance when required by the applicable program. Participate in chart reviews, audits, monitoring, quality assurance, and quality improvement activities. Correct identified documentation or compliance deficiencies within established timeframes. Prepare and submit required reports, information, and program data as assigned. Minimum Qualifications Bachelor's degree in Social Work, Human Services, Public Health, or a related field required; a degree in Social Work is preferred. Minimum of 2 years of case management or social services experience preferred. Experience working with vulnerable or underserved individuals and families strongly preferred. Experience in community-based services, healthcare, housing, public health, social services, or grant-funded programs preferred. Valid driver's license and ability to travel as required. Technology Skills Candidates must possess the basic technology skills necessary to independently perform the responsibilities of the position, including: Working proficiency with Microsoft 365, including Outlook, Teams, Word, Excel, and PowerPoint. Ability to use professional email, electronic calendars, virtual meetings, and shared electronic files. Ability to create, edit, organize, save, and share electronic documents. Ability to enter, organize, review, sort, and filter basic information in Excel. Ability to complete electronic forms and maintain electronic client records. Ability and willingness to learn and effectively use HMIS, CAREWare, electronic case-management systems, and other software required by ASC or assigned programs. Work Environment This position may require frequent local or regional travel based on program and client needs. Case Managers may conduct home and community-based visits in a variety of environments, and occasional evening or weekend work may be required. ASC serves individuals and families who may be experiencing significant health, housing, financial, behavioral health, social, or other life challenges. Case Managers must be able to work respectfully and effectively with individuals experiencing complex circumstances while maintaining professionalism, confidentiality, appropriate boundaries, compassion, and sound judgment. Performance Expectations Success in this position requires consistent ownership of assigned responsibilities and caseloads. Performance will be evaluated based on effective caseload management; quality and timeliness of work; client engagement and follow-through; accuracy and completeness of documentation; compliance with applicable program requirements; progress toward client and program outcomes; communication and teamwork; and the ability to consistently carry out assigned responsibilities. Supervisors provide program oversight, guidance, technical assistance, and support with complex situations. However, Case Managers are expected to independently organize, monitor, and complete the routine day-to-day work associated with their assigned caseload. Why Join ASC?
