A Clinical Case Manager coordinates medical care and provides advocacy to help people get the best and most affordable care. Helps patients navigate insurance and hospital bureaucracies and access information. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment and travel to different doctors' offices, hospitals or a patient's home.
The Community Case Manager in PATH assists individuals who are homeless and experiencing severe and persistent mental illness toward acceptance of mental health and other critical services as well as in housing stabilization in the community. This is accomplished using a holistic approach that focuses on immediate, intermediate, and long-term needs. Essential Functions Engage and conduct interventions with individuals and their families. Responsible for completing assessments, rapport building, skill development, symptom management, housing attainment, critical service need identification, service utilization, and recovery planning (65%ofstafftime, inclusive of coverage and travel time). Assess individual's appropriateness for program services. Documents all clinical activity and correspondence in accordance with Center policy , state regulations, and Joint Commission standards. Participates in team meetings and supervision. Collaborates with other treatment providers to monitor and discuss clients' progress and needs.
Performs other related duties as assigned. Qualifications Essential Master's Degree in Social Work, Counseling Psychology, Rehabilitation Counseling or Psychiatric Nursing from an accredited college/university Must possess a valid NJ driver's license in good standing. Minimum of two-year experience with adults with severe mental illness (SMI) Has experience in working across cultures. Skill in the operation of office equipment, including but not limited to computers, printers, copiers, calculators, and other job specific equipment. Ability to work collaboratively and build relationships with colleagues, coworkers, vendors, contractors, and the public. Ability to accurately prepare and maintain files, records, reports, and logs. Skill in using computer applications including electronic health record, spreadsheets, databases, and word processing software. Ability to identify and maintain confidential information.
Ability to comprehend, express and exchange information in English over the phone, in person, in writing or through other communication or electronic devices. Preferred Bilingual ability (English/Spanish) Knowledge of Wellness and Recovery Principles, Person Centered Care, Trauma Informed Care and Integrated Healthcare.
Lived experience in mental health and/or substance use disorders in recovery for a minimum of five years.
Hours:
Full time: 40 hours/week Salary range: $55,000 - $60,000/year Benefits include health coverage (medical, prescription, and dental), paid time off (vacation, holidays, and sick), a deferred compensation plan, and enrollment in the NJ State Pension System.
