Carmel, NY Apply Description 2 POSITIONS available - Schedule Sun-Thursday 2pm -10:30pm and Tuesday-Saturday 2pm-10:30pm
FUNCTION
To provide comprehensive clinical case management services that include: assessments; individual, group and family treatment; treatment plan coordination; referent relations; discharge planning; and intervention with client support systems, including social service programs and managed care.
ESSENTIAL DUTIES & RESPONSIBILITIES
include the following. Other duties may be assigned. 1. Coordination of census. 2. Coordination of patient transfers. 3. Overseeing the caseloads and scheduling of the Adult Program Clinical Case Managers and attend team meetings and audit paperwork as needed.
ASSIST PROGRAM DIRECTOR WITH
4. Overseeing treatment planning. 5. Evaluation and delegation of rules for the Program as put forth by management. 6. Addressing all conflict issues with both staff and patients. 7. Conducting wing meetings. 8. Reviewing and modifying the program's services. 9. Public relations with other treatment modalities to assess appropriate referral for adult clientele. 10. Maintaining Coordinator's own reduced case load and responsibilities as outlined in Case Manager (CASAC/LMSW) job description relative to licensure and title. 11. Participates in Performance Improvement Initiatives. #INDHP Requirements
