N/A Job Summary Coordinates the care and services of patients across the continuum of their hospitalization. The case manager assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors, and measures the effectiveness of interventions to meet patient's treatment and discharge needs, which would achieve optimal clinical and resource outcomes. Qualifications Graduate of NLN Accredited School of Nursing BSN Preferred or Masters Degree from an accredited school of Social Work 1-3 years experience in an acute care hospital Ability to read, write, and communicate in English. Strong clinical, organizational, and interpersonal skills. Current New York State Licensure as a Registered Professional Nurse. Required Licensed Clinical Social Worker - New York State Board of Social Work within 180 Days Required Certification in Case Management required 18 months after eligibility PRI certification Preferred Montefiore St. Luke's Cornwall is an equal employment opportunity employer.
