- New York State Board of Nursing Current New York State Licensure as a Registered Professional Nurse. Required Licensed Clinical Social Worker
- New York State Board of Social Work within 180 Days Required CCM
- Certified Case Manager
- Commission for Case Manager Certification Certification in Case Management required within 1-1/2 Yrs Required PRI
- Patient Review Instrument Cert
