Job SummaryHamilton Health Center is seeking an Intensive Case Manager I to provide comprehensive case management, care coordination, and advocacy for individuals receiving recovery and supportive services. In this role, you will work closely with clients, healthcare providers, and community partners to coordinate services, reduce barriers to care, and support each client's recovery journey through individualized service planning and ongoing care coordination. QualificationsRequiredBachelor's degree in Human Services or a related fieldStrong interpersonal, communication, and organizational skillsBasic computer and data entry proficiencyFundamental understanding of healthcare services and medical terminology PreferredCPR and/or First Aid certificationOne (1) year of experience in a medical or physician office settingKnowledge of Joint Commission standardsBilingual in Spanish, Nepali, or French Work EnvironmentOffice and community-based position supporting clients throughout the service areaFrequent interaction with healthcare providers, community organizations, and client support systemsMay involve transportation assistance and community outreach activitiesExposure to routine medical office and clinic environments while supporting individuals with behavioral health and recovery needs.
