Job Description

Job Summary Hamilton Health Center is seeking a Medical Case Manager to provide specialized case management services to individuals living with HIV through our HOPE program under the Ryan White Part B program. This role plays an important part in improving health outcomes by helping clients access appropriate medical care and support services, promoting treatment adherence, and supporting continuity of care. The Medical Case Manager will work collaboratively with an interdisciplinary team to provide patient-centered services while ensuring accurate documentation and compliance with applicable program, funding, and regulatory requirements. Why Join Hamilton Health Center Mission-driven organization serving individuals and families throughout Central Pennsylvania Opportunity to make a direct impact in the lives of individuals living with HIV Collaborative, interdisciplinary care environment Opportunity to connect clients with healthcare, supportive services, and community resources Stable, community-focused work setting Compensation & Benefits Generous paid time off Birthday holiday + 7 paid holidays Medical, dental, and vision coverage Company-paid life insurance Retirement plan with employer match Employee Assistance Program Key Responsibilities Provide specialized medical case management services to individuals living with HIV Facilitate timely and coordinated access to appropriate medical care and support services to promote continuity of care Conduct treatment adherence activities to assess client readiness for and adherence to HIV treatment Maintain compliance with documentation, reporting, and client contact requirements established by program funding organizations and regulatory bodies Enter required documentation accurately and efficiently into appropriate electronic systems Collaborate with interdisciplinary teams to support a patient-centered approach to improving health outcomes Conduct rapid HIV testing in both office and community-based settings Participate in outreach activities, community events, and patient engagement initiatives Complete and maintain required continuing education in accordance with program regulations Qualifications Required Knowledge of case management principles and functions within a healthcare setting Proficiency in data entry with the ability to learn and utilize program-specific systems and databases Strong interpersonal and communication skills with the ability to engage individuals from diverse backgrounds Demonstrated cultural humility and understanding of diverse populations and health disparities Strong organizational skills with attention to detail and compliance requirements Ability to work collaboratively with clinical teams, leadership, and community partners Valid driver's license with the ability to travel throughout the service area Preferred Bachelor's degree in Social Work, Psychology, Sociology, or a related field Two (2) years of experience in case management or a comparable social services role Experience working with individuals living with HIV and conducting rapid HIV testing in community settings Knowledge of community resources in Harrisburg and surrounding areas Bilingual (Spanish) Work Environment Work is conducted in medical office/health center and community-based settings Regular interaction with patients, clinical teams, community partners, and members of the public Travel throughout the service area is required May involve exposure to routine healthcare conditions, including infectious or contagious diseases All About Hamilton Health Center Hamilton Health Center (Hamilton), established in 1969, is a Federally Qualified Health Center (FQHC) within Harrisburg, PA and continues to grow using a holistic and comprehensive approach to being patient centered.
