A Clinical Case Manager coordinates medical care and provides advocacy to help people get the best and most affordable care. Helps patients navigate insurance and hospital bureaucracies and access information. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment and travel to different doctors' offices, hospitals or a patient's home.

Job Description

We are looking for people who want to make their mark on the world! The LCPC or LCSW meets the individual's presented clinical needs to include assessment, brief intervention or long term therapy as indicated, as well as minimal crisis support. In-person, hybrid, and fully remote job opportunity
Essential Duties and Responsibilities:
  • Provides clinical assessments
  • Analyzes patient health needs and establishes treatment plan
  • Provide individual, with potential for group treatment
  • Provides crisis intervention to align with clinical standards
  • Makes referrals to internal/external programs as appropriate
  • Provide proper clinical documentation
  • Work within the KEPRO system for MH programs
  • Documentation tracking for MH programs
  • Communicates with clinical, medical, and administrative staff
  • Performs other duties as assigned/necessary
Minimum Requirements:
  • Current license as a LCPC or LCSW in the state of Maine, in good standing. Conditionally licensed individuals encouraged to apply!
  • Experience in behavioral health guidelines, to include behavioral health assessments, individual and group therapy, clinical management of patients with mental illness and / or substance abuse issues preferred but not required
  • Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills
  • Computer proficiency required Benefits
  • 2 weeks paid vacation/year accrued (payable at hours typically worked per week) to start
  • 6 sick days per year
  • 2 personal days per year
  • Weekly or biweekly clinical supervision, depending on license level and individual needs
  • Employee contribution of $300/month toward qualifying health insurance plan
Pay:
$35.00 - $55.00 per hour
