Develop individualized care plans that incorporate medical treatment, social services, and community resources. Coordinate with hospital staff, including nurses, physicians, social workers, and specialists such as ICU or trauma center teams, to facilitate optimal patient transitions. Monitor patient progress through regular follow-up contacts and adjust care plans accordingly to ensure continuous improvement. Collaborate with healthcare providers on utilization review and management to optimize resource use while maintaining quality standards. Facilitate discharge planning by arranging necessary services such as hospice care, behavioral health support, or outpatient therapies. Maintain detailed documentation within EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems in compliance with HIPAA regulations. Communicate effectively with patients and their families across language barriers; bilingual skills are highly valued. Ensure adherence to healthcare policies including HIPAA, managed care protocols, and medical coding standards such as CPT (Current Procedural Terminology), ICD-9/10 (International Classification of Diseases), and ICD coding practices. Work closely with social services teams to connect clients with community resources and support systems tailored to their cultural backgrounds.
