As a Referral Case Manager , you'll play a vital role in connecting patients with specialty services, coordinating care, and ensuring seamless communication between providers, specialists, and patients. Your Responsibilities Monitor and process referrals generated by MyCHN providers on a daily basis. Review referral information for accuracy, completeness, and medical necessity. Coordinate specialty referrals according to established guidelines and provider availability. Educate patients on referral status, appointment scheduling, and next steps in their care journey. Assist patients in overcoming barriers to specialty care, including transportation, insurance verification, and appointment coordination. Communicate effectively with providers, specialists, and internal clinical teams to ensure continuity of care. Maintain accurate documentation of referral activities and patient interactions within the Electronic Health Record (EHR). Track referral outcomes and ensure appropriate follow-up. Escalate urgent or complex cases to clinical leadership when necessary. Support quality initiatives and maintain compliance with organizational policies and HIPAA regulations. Participate in team meetings and contribute to workflow improvement initiatives.
