Care Transition Associate Multiple Locations:
Mission Hills, CA Mission Viejo, CA Orange, CA Torrance, CA Position Overview The Chart Specialist / Intake Coordinator performs patient-centered administrative tasks to support home health care transitions and care coordination. Working on-site within acute care hospital hubs, this role audits patient charts, enters critical medical data from Epic into the Compassus EMR system, and places provider verification calls to facilitate smooth transitions to post-acute care. Responsibilities Audit patient charts provided by hospital liaisons for accuracy prior to transitioning patients to Home Health care. Transfer medical chart data directly from Epic into the Compassus EMR system with high precision. Place confirmation calls to verify primary care providers (PCP) for incoming Home Health patients. Process 15-25 charts and handle corresponding call volumes daily across assigned California facilities. Act as a non-clinical liaison between inpatient and outpatient settings, coordinating post-acute care arrangements, DME/HME vendor deliveries, and discharge documentation. Provide administrative support to Clinical Liaisons, hospital discharge planners, and physician offices while maintaining strict HIPAA compliance. Required Qualifications 1+ years of recent experience in medical records, healthcare insurance, or a healthcare administrative support environment. Associate degree OR a High School Diploma/GED with 3+ years of equivalent healthcare administrative work experience. Strong technical proficiency and accurate data entry skills, including experience navigating EMR systems. Proven critical thinking, fast decision-making, and organizational skills under fast-paced demands. Excellent written and verbal English communication skills, punctuality, and a strong work history/job tenure. Preferred Qualifications Experience with Epic electronic medical records (EMR) software. Prior background as a Clinic Manager for a small practice or in healthcare administration. Coursework or training in medical terminology and healthcare insurance systems. Proficiency in MS Word and Excel. Schedule 8:30 AM
