Clinical Consultant - Per Diem The Clinical Consultant collaborates with the Case Manager and other team members as a part of a multi-disciplinary Care Management Team providing consulting services of the highest quality to the clients of 1 Link 4 Senior Care, Inc. and its divisions (1 Link 4 Care and 1 Link 4 Pride). Clinical Consultants are an essential part of our multi-disciplinary Care Management Team where they use their specialized skill set to collaborate on and provide the best possible solutions for our clients' unique needs, working to enhance treatment plans that optimize care and improve the client's quality of life.

Key Responsibilities:

Conduct visits to the home, hospital, or long-term care facilities Assess client's physical, mental, and emotional state of health Evaluate client living environments for safety and sustainability of aging in place Provides education and recommendations to clients and their caregivers, and when needed, provide clinical updates to the client's PCP and other specialists The Ideal Candidate for
