This role is dedicated to strategic outreach efforts prioritized to address screening/preventative gap closure needs which align with value-based partnerships. It involves organizing, engaging, and tracking outreach efforts that promote health and wellness in the community. This role manages the workflow and support of primary care provider (PCP) assignment/reassignment with health plan partners. This position must demonstrate exceptional customer service skills and professionalism. This position is required to be proficient in reading/writing/speaking Spanish.

Education and Experience:

High School Diploma or GED required. One year of relevant experience is preferred. Must be able to read, write, and speak Spanish fluently. Experience with healthcare related outreach calls, managing/tracking healthcare related data, and/or experience with an Electronic Medical Records (EMR) system preferred.
