Responsibilities Deliver patient personal care assistance, including helping with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and patient transfer methods. Monitor vital signs regularly and observe patients for changes in condition or behavior, especially those with dementia, Alzheimer's disease, or neurodevelopmental disorders. Administer medications accurately following care plans while ensuring compliance with HIPAA regulations to protect patient privacy. Assist with meal preparation and feeding for residents requiring nutritional support or specialized diets related to medical conditions like diabetes or dysphagia. Support residents with memory care needs through specialized techniques tailored for dementia and Alzheimer's care; maintain detailed documentation of patient progress and care plans. Provide transportation assistance for appointments or community outings using personal transportation methods safely and responsibly. Maintain a clean and safe environment by performing routine cleaning tasks and adhering to infection control protocols essential in home health, hospice care, or senior living settings. Engage actively in patient observation and communication to ensure holistic support that respects individual preferences and needs. Skills Proven experience working with individuals with disabilities, developmental disabilities, or neurodevelopmental disorders such as autism. Strong knowledge of patient monitoring techniques, medication administration, first aid, CPR certification, and manual handling procedures including heavy lifting. Familiarity with care plans development and implementation for residents with dementia or memory impairments; experience in Alzheimer's care is highly valued. Excellent communication skills to effectively interact with clients, families, healthcare teams, and social workers while maintaining confidentiality per HIPAA standards. Ability to perform assisted living tasks such as assisting with ADLs, personal transportation assistance, and supporting residents' social engagement activities. Experience working in home health environments or long-term care facilities like nursing homes; familiarity with hospice care is a plus. Competence in cleaning routines necessary for maintaining sanitary conditions within healthcare settings; knowledge of safety protocols related to manual handling and patient transfer methods.
