Position Details:
As a Supervisor of Integrated Health & Social Care, you'll lead a team of Community Health Workers while also helping patients navigate healthcare and community resources. In this role, you'll: Lead, coach, and develop a team of Community Health Workers Support uninsured and underinsured patients in accessing healthcare services and financial assistance programs Connect patients with resources that address social needs such as transportation, food access, and financial challenges Help patients manage chronic conditions and achieve their health goals Build partnerships with community organizations and collaborate with Mercy leaders to improve health outcomes This is a rewarding opportunity for someone who is passionate about leadership, patient advocacy, and making a meaningful impact in the community by removing barriers to care and helping patients access the support they need to thrive.
Minimum Required Qualifications:
Education :
Bachelor's degree in community health, public health, or other with related coursework
Experience :
3 or more years CHW experience or equivalent
Certification/Registration:
Certified in at least one chronic disease certification program (i.e. DPP) and
Basic Life Support Other Skills and Knowledge:
Epic electronic medical record, reporting, dashboards
Preferred Qualifications:
Certification/Registration:
Community Health Worker Certification if available
Other Skills and Knowledge:
