Duties Assist clients with daily activities, ensuring safety and promoting independence in accordance with individualized care plans Facilitate group sessions, workshops, and community outreach activities focused on health education, nutrition, addiction counseling, and wellness strategies Support program development efforts by collecting data, preparing reports, and maintaining accurate electronic health records (EMR/EHR) Engage with culturally diverse communities through outreach initiatives and community engagement strategies Utilize motivational interviewing techniques to encourage behavioral change and enhance client motivation Provide basic life support (BLS) when necessary and respond effectively to emergency situations Communicate clearly with clients about health information, public health policies, Medicare options, and available resources Skills Strong background in nursing, social work, or related healthcare fields with experience working with individuals with disabilities or from underserved communities Proficiency in Microsoft Office Suite for documentation, reporting, and program planning purposes Knowledge of public health law policies, community health practices, and public health program planning Excellent written and verbal communication skills for client interaction and public speaking engagements Ability to work effectively with culturally diverse populations using bilingual skills when applicable Experience in community outreach, engagement strategies, and data collection methods Familiarity with medical terminology, nutrition principles, addiction counseling techniques, and Medicare regulations Demonstrated ability to develop programs that promote health education and community involvement Join us in making a positive impact!
