is a whole-person, interdisciplinary approach to comprehensive care management that addresses the clinical and non-clinical needs of high-cost, high-need managed care members through systematic coordination of services that is community-based, interdisciplinary, high-touch, and person-centered.CONDITIONS OF EMPLOYMENT
Background:
Prior to appointment, applicants will be fingerprinted and must pass an extensive background investigation, including a polygraph examination. Applicants must not have been convicted of a felony.Travel:
Travel throughout the County may be required and employees will be required to make provisions for such transportation.Basic Life Support Certification:
Positions within Clinical Operations may require a Healthcare Provider level Basic Life Support (BLS) certification.CHW Certification:
Incumbents must successfully complete a Community Health Workers (CHW) Certification program resulting in a certificate within one (1) year of hire or be terminated.Sponsorship:
