$43,000 - $46,000 Annual base

SUMMARY:

The Community Health Outreach Worker/Promotora for the FIT engages community members through outreach to reduce stigma and provides workshops on wellness and mental health topics. This position actively engages with vulnerable communities, including Immigrant and Agricultural workers, addressing various health and environmental concerns that influence their well-being. They provide outreach, training, and resources related to mental health, health education, vaccination awareness, public health issues, and environmental health concerns. They also support initiatives while participating in community and youth events that promote health, well-being, resiliency, and access to resources. The CHW/Promotoras also serves as a liaison between clients and the mental health clinical team, following up on referrals, coordinating with clinicians. This position works collectively as part of a team as well as independently.

MAJOR DUTIES AND RESPONSIBILITIES

Provide culturally appropriate services and resources to targeted communities. Conduct outreach efforts on specific health and wellness initiatives. Develop and deliver workshops, resource materials, and case management support as needed. Actively participate in community and youth activities and events. Promote the medi-cal members health or address barriers to health care Health navigation, provide information, training, referrals, or support to assist medi-cal members

Screening and Assessment:

assist medi-cal members to connect to appropriate services to improve their health Individual support and advocacy Represent the program in meetings with committees, community agencies, and related entities. Follow up on mental health referrals to ensure clients are connected to appropriate services. Assist clinicians in preparing materials and logistics for group counseling sessions. Monitor client progress in collaboration with clinicians and provide feedback to the mental health team. Maintain confidentiality and adhere to HIPAA and agency privacy policies. Will understand the differences between billable services versus not billable and document appropriately. Maintain good management skills, to ensure following policies around timeliness and documentation. Other duties as assigned.

EDUCATION/EXPERIENCE REQUIREMENTS

Community health worker certification/ Lived experienced Demonstrated experience working with cultural communities. Experience working with multicultural and diverse populations. Proficiency in utilizing technology to prepare and track deliverables.

QUALIFICATIONS

Strong attention to detail, ability to prioritize, and manage multiple tasks. Effective problem-solving skills, ability to meet deadlines, and adapt to changing circumstances. Cultural awareness regarding race, ability, sexual orientation, gender identity and other social identities, with an existing ability to communicate and collaborate with people across lines of difference with respect and dignity. Must provide linguistically and culturally sensitive care to promote the reduction of cultural barriers to all the community without bias. Must have a cultural understanding and able to work with a diverse population, including but not limited to language proficiency. Ability to provide linguistically and culturally sensitive care, reducing cultural barriers within the community without bias. Demonstrated cultural understanding and ability to work with a diverse population, including language proficiency. Must possess a valid California Driver's License and vehicle insurance. Preferably bilingual in Spanish.
