Northern California Starting Pay:
$25.00 per hour Full-Time Hiring Immediately About Us BLISS (Better Living in a Sociable Society) is a nonprofit organization dedicated to improving access to healthcare, community resources, and supportive services for individuals and families throughout Northern California. We are looking for a compassionate, motivated Lead Community Health Worker (CHW) to help us expand our impact. Position Summary The Lead Community Health Worker will provide community outreach, connect clients with essential services, assist with care coordination, and help support Community Health Workers in the field. This role is ideal for someone who enjoys working directly with the community and making a meaningful difference every day. Minimum Qualifications High School Diploma or GED (Required) Minimum of 1 year of Community Health Worker (CHW) or related community outreach experience Reliable transportation Valid driver's license and current auto insurance Strong communication and interpersonal skills Ability to work independently and as part of a team Responsibilities Conduct community outreach and client engagement Complete client assessments, documentation, and follow-up Connect clients to healthcare, housing, food, transportation, and community resources Coordinate services with community partners Maintain accurate records and documentation Assist with outreach events and community education Support and mentor Community Health Workers as assigned Preferred Skills Knowledge of local community resources Experience working with underserved populations Professional, compassionate, and dependable Strong organizational and time management skills
Compensation Starting Pay:
$25.00 per hour Opportunities for advancement Ongoing training and professional development Meaningful work serving Northern California communities Work Location Positions are available throughout Northern California.
