Are you ready to make a difference: The Community Health Worker helps rural residents access nutrition, wellness, healthcare, and community resources through the Northeast Rural Nutrition & Wellness Initiative. This position provides outreach, enrollment, wellness screenings, nutrition education support, food assistance navigation, referrals, follow-up, and participant engagement. The role is funded through the RHTP grant and supports program documentation, referral tracking, and outcome reporting.

• Credentials, Experience, and Abilities Ÿ High School Diploma or equivalent required; associate or bachelor's degree in health, human services, nutrition, social services, or related field preferred. CCHW Certified Community Health Worker preferred or in certification process. Ÿ Experience working with rural, low-income, underserved, or medically vulnerable communities preferred. Ÿ Fulfill credentialing requirements by agency policy. Ÿ Ability to document participant engagement, referrals, follow-up, and outcomes accurately in SmartCare, Health Information Exchange systems, and other required program records. Ÿ Knowledge of community health, nutrition education, food assistance programs, wellness screenings, referrals, and community resource navigation preferred. Ÿ Possesses initiative and strong organizational skills with the ability to work effectively and ensure confidentiality. Ÿ Administrative self-sufficiency including computer proficiency. Ÿ Possesses valid driver's license and safe driving record. Must meet insurability standards in order to drive. Normal Working Hours and Conditions Ÿ 40-hour workweek for full-time Community Health Worker positions or as otherwise assigned based on program needs and grant requirements. Ÿ Weekends, evenings, outreach events, community-based activities, and participant follow-up may be required. Ÿ Travel throughout eligible rural communities in the Northeast Region and to partner/community locations may be required. Ÿ Able to lift and carry 25 lbs.
