Preferred Qualifications:

CHW certification is preferred but not required for qualified entry-level candidates. Candidates without certification must be willing to complete the required training and certification pathway.

Job Description:

The Community Health Worker is a full-time, field-based position serving older adults throughout Central Florida through a structured 30-day post-hospital discharge program. CHWs conduct home visits, complete participant assessments, provide health education and non-clinical emotional support, assist with healthcare and social service navigation, accompany participants to medical appointments when needed, coordinate transportation, meals and approved services, provide basic home cleaning assistance, connect participants with appropriate resources, and complete required case notes and program documentation. CHWs work independently with an assigned caseload while collaborating with supervisors, healthcare providers, social workers, caregivers, and community partners. Job Salary:

$18.00 per hour Mileage reimbursement: $0.445 per mile

Job Schedule:

Monday through Friday, 9:00 AM to 5:30 PM, 40 hours per week. Field-based throughout
