The Fulton County Board of Health provides a variety of services that help protect residents from health threats, increase access to health services to improve health outcomes, and provide information that assists Fulton County citizens in living healthier lives. The agency is seeking a highly qualified candidate for the position of Community Health Worker within the Fulton County Board of Health in the Chronic Disease Prevention Program. Position Description Under direct supervision, the Community Health Worker (CHW) plays a vital role in helping individuals and families navigate and access community services, health resources, and adopt healthier lifestyles. This position emphasizes community outreach and engagement, including participating in events, delivering health education, building trust with community members, and promoting preventive care. The CHW also provides one-on-one support and advocates for the social and healthcare needs of clients to help improve overall health outcomes. Responsibilities Provide health coaching and social support to encourage clients in managing chronic conditions, seeking care, and adopting healthier behaviors. Help community members get appropriate screenings, referrals, and linkages to service for health and social services. Participate and lead community outreach events and health education activities. Facilitate community conversations to raise awareness and address local health needs. Distribute written materials and resources to promote outreach initiatives. Attend regular staff meetings, trainings, and relevant professional development events. Conduct educational presentations and other outreach efforts targeting priority populations. Maintain up-to-date knowledge of community resources and public health strategies through participation in required training and conferences. Serve a non-traditional flex schedule, evenings and some weekends and holidays. Position Requirements Excellent customer service skills. Experience working in community-based settings or with diverse populations is strongly preferred. Reliable personal transportation and a valid driver's license. Self-directed with excellent organizational skills. Strong communication and interpersonal skills with a passion for helping others. Ability to work independently and as part of a team. Familiarity with community resources and the ability to make appropriate referrals. Minimum Qualifications Bachelor's degree in a related field and one year's experience performing work related to area of assignment. Preferred Qualifications In addition to the minimum qualifications, preference will be given to applicants with experience in public health education, community outreach, and knowledge of socially and economically disenfranchised, racial/ethnic minorities, those who have chronic conditions or are at risk of chronic disease and other health and social service-related needs.
