Why This Role Matters As a Community Health Worker, you will serve as a bridge between patients, healthcare providers, and community resources. You'll help individuals navigate complex healthcare systems, overcome social and economic barriers, and gain access to services that improve their overall well-being. Your work won't just impact individual patients, it will strengthen entire communities. Whether you're helping someone find transportation to a medical appointment, connecting a family with food assistance, educating patients about chronic disease management, or advocating for community needs, you'll play an essential role in improving health outcomes and creating lasting change. What You'll Do Build trusting relationships with patients and families Connect individuals to healthcare services and community resources Help address social drivers of health such as food insecurity, transportation, housing, and access to care Provide health education and support in a culturally sensitive manner Collaborate with care teams to improve patient outcomes Support quality improvement initiatives and patient-centered care efforts Advocate for the needs of patients and communities Who Thrives Here?
