You'll help individuals navigate complex healthcare systems, overcome social and economic barriers, and gain access to services that improve their overall well-being. What You'll Do Build trusting relationships with patients and families Connect individuals to healthcare services and community resources Help address social drivers of health such as food insecurity, transportation, housing, and access to care Provide health education and support in a culturally sensitive manner Collaborate with care teams to improve patient outcomes Support quality improvement initiatives and patient-centered care efforts Advocate for the needs of patients and communities Who Thrives Here?
