Under supervision, the Community Health Worker I (CHW I) provides health education, disease management education, health care and social service navigation services, and/or case management services to patients/community residents

MINIMUM EDUCATION

 High school diploma or

GED. MINIMUM WORK EXPERIENCE

 One year of health education, health care, or related experience preferred.

KNOWLEDGE & SKILLS

 Ability to provide health education, one-on-one and in groups.  Ability to provide case management under supervision.  Familiarity with and/or experience with working in Chicago's West Side and/or South Side communities (resident of West Side or South Side communities preferred).  Passion for and dedication to working with community members and providers.  Ability to connect and build trust with diverse patient/participant population.  Demonstrated commitment to racial and health equity.  Punctual, reliable, and willing to learn.  Working understanding of Microsoft Word, Access, Excel, and PowerPoint preferred.  Valid driver's license and own car preferred (required for some positions).  Bilingual in Spanish may be required for some positions.

• Hours are flexible but will include some evening and weekend hours scheduled in advance. .
