This role helps ensure that community resource information is accurate, current, and easy for Patient Navigators and Community Engagement Specialists (CES) to use when connecting patients to services. The Associate conducts basic community research, communicates with community organizations, maintains resource information, and supports the use of the BigHeart platform. The role works closely with internal teams and community partners to help expand awareness and access to resources addressing Social Determinants of Health (SDOH), including food, housing, utilities, transportation, and income support. This is an entry-level full-time position for an organized, dependable, and service-oriented individual who is comfortable using and learning new technology, communicating with a variety of organizations, and managing information accurately. Key Responsibilities 1. Community Resource Research & Outreach Research nonprofit, faith-based, community-based, and government organizations that provide social services to individuals and families. Identify community resources based on established Social Determinants Of Health categories and geographic areas. Gather and document basic information about organizations, including services offered, contact information, eligibility requirements, hours, and service locations. Contact community organizations by phone, email, or virtual meeting to verify information and introduce the BigHeart platform. Maintain professional and positive relationships with community organizations. Follow established outreach and resource verification processes. Escalate resource gaps, challenges, or questions to the appropriate team member. 2. Resource Database Management Enter and update community organization and resource information in the BigHeart platform and other designated systems. Regularly review resource information to identify outdated or incomplete information. 3. Internal Team Support Provide Patient Navigators and CES teams with basic information about available community resources. Help identify appropriate resources based on established referral guidelines and patient needs. Respond to routine questions about resource information and refer more complex questions to the appropriate team member. Share updates when community organizations add, change, or discontinue services. Support the preparation of resource lists, reports, presentations, and other materials. 4. Training & Resource Materials Help support training activities for Patient Navigators, CES team members, and community partners, as directed or as needed. Assist with preparing simple job aids, resource guides, reference materials, and presentations. Participate in community partner meetings, training sessions, and team meetings. Learn and stay current on BigHeart platform features and established resource management processes. Required Qualifications High school diploma or equivalent; associate degree or relevant coursework preferred. Interest in community services, healthcare, social services, nonprofit organizations, or a related field. Proficiency with Google Workspace , including Google Sheets, Google Docs, and Google Slides. Strong written and verbal communication skills. Strong organizational skills and attention to detail. Ability to conduct basic online research and gather information from multiple sources. Comfortable communicating with individuals from diverse backgrounds and organizations. Ability to learn and use new technology and software platforms. Ability to manage multiple tasks, follow established procedures, and meet deadlines. Verify contact information, services, eligibility requirements, hours, locations, and referral instructions. Track outreach activities and the status of community organization engagement. Organize information so Patient Navigators and CES team members can quickly find and understand available resources. Follow established procedures for maintaining data quality, confidentiality, and accuracy. Ability to work independently while knowing when to ask questions or seek assistance. Preferred Qualifications Experience using databases, CRM systems, referral platforms, or other web-based software. Experience making professional phone calls, sending outreach emails, or coordinating with community organizations. Familiarity with Social Determinants of Health (SDOH) or community resources. Experience working with underserved or diverse communities. Experience volunteering or working with a nonprofit, community-based, faith-based, healthcare, or social service organization.
Pay:
From $17.00 per hour Expected hours: 40.0 per week
