Pediatric Job Description Overview Location:
Riley Hospital Hours:
40 hrs/wk Monday-Friday 8am-5pm with the ability to work evenings and weekends for special events or community outreach activities. Serves as a bridge between the community and the health care, government and social service systems. Assists individuals, families, groups and communities access resources, including health insurance, food, housing, quality care and health information. Provides informal counseling, support, and follow-up.
Key Responsibilities:
Assess patients' and families' immediate needs, risks, and available support systems Communicating with licensed clinical social workers and community partners to coordinate resources and services Maintain accurate and timely documentation Provide informal counseling, support, and ongoing follow-up as appropriate
Preferred Qualifications:
Interest and compassion in supporting individuals and families impacted by violence Experience working in a healthcare setting strongly preferred Strong interpersonal, communication, and advocacy skills Ability to build rapport with patients, families, and community organizations in a compassionate and professional manner
Requirements:
High School Diploma or equivalent required. 0-3 years of experience required. Requires basic computer skills Requires knowledge of medical, community, and social resources.
